Template — Access not met decision — Post-

legislation changes

This template will help Access Assessors update the Letter — Access not met decision to explain the reasons they decided an applicant is not eligible for the NDIS.

This includes the following decision reasons:

  • age requirements not met
  • residence requirements not met
  • disability requirements not met
  • early intervention requirements not met.

Note: Use this template to update letters for post-legislation change decisions. For pre- legislation change decisions, go to [Template — Access not met decision — Pre-legislation changes](Template — Access not met decision — Pre-legislation changes).

3. Templates

Template content has been ‘hidden’ to make it easier to find the correct template. When you find the template you need, reveal the content by:

  1. Selecting the arrow next to the heading, or

  2. Right-click the heading then:

    • select Expand/Collapse
    • select Expand Heading.
  3. Copy template into a blank Microsoft Word document.

  4. Customise the template using content control fields including: text fields, drop-down menus (labelled as ‘Choose an item.’) and date fields.

  • To avoid missing any fields, you can use the Tab button on your keyboard to move from one field to the next.
  • Check any title in the field for instructions about what to write. For example, it might prompt you to choose between ‘you’ or ‘they,’ depending on who will receive the letter:
