Complete the Form – Evidence of developmental delay (EODD)

Guidance in this document is not approved for use unless you view it in PACE.

This article provides guidance for an early childhood partner to:

• complete the Form – Evidence of developmental delay (fillable PDF) when there are

concerns about delay(s) in the development of children younger than 6 (who do not have

any evidence of permanent impairment) as part of the community connections case

• understand qualifications required to complete and sign the EODD form.

Complete the Form - Evidence of developmental delay

Section 1: Contact details

Record the relevant details.

Section 2: Supporting information (if available)

Record the details of any additional sources of information (only if available – not mandatory) and refer to article Complete the DC/DD (developmental concern/developmental delay) tab for process steps on how to upload supporting information.

Section 3: Early childhood partner observation(s) and assessment(s)

Gather information using a combination of the following methods, ensuring there is sufficient evidence to support and proceed with the child’s access request if this is the recommendation for the child.

• any additional assessments and screening tools that you may have completed (for

example: ASQ:SE-2, YC-PEM, PEM-CY or BRIGANCE) completed. To upload any

supporting information, refer to article Complete the DC/DD (developmental

concern/developmental delay) tab which outlines the process steps to upload supporting

documents

• whether an observation of the child, including how they interact with parents/family and

others, was completed

• where the observation(s) took place

Section 4: Impairments (body functions and body structures)

Record on the form:

• whether the child has a mental or physical impairment, or a combination of both

• the mental or physical impairment(s) identified

• the major life activities areas, if any, that are impacted by the impairment(s).

Section 5: Functional capacity in major life activity areas

Describe the child’s current abilities and any delays in development across each area of major life activity:

• self-care

• receptive and expressive language

• cognitive development

• motor development

These are the relevant domains in the legislation. If the child is typically developing in any of the major life activity areas, provide a brief summary of the child’s functional capacity and go to the next major life activity area.

If there are concerns about the child’s development in any major life activity areas, describe their functional capacity in each sub-section and record whether substantially reduced functional capacity is identified. Use all the information gathered and your professional opinion to complete this task. Use concise descriptions and avoid long narratives. Bullet points are acceptable.

Following the completion of each sub-section you will need to provide your overall professional opinion about the likelihood that the child meets substantially reduced functional capacity criteria in each major life activity area.

Section 6: Service and support requirements

This section is used to evidence your recommendation for the most appropriate service and support requirements for the child based on the delays in their development and whether the child is likely to meet the early intervention requirements - developmental delay (external).

We need to know that the child needs a mix of specialist care, treatment or other services, due to their developmental delay. The child must also need these services for an extended duration – that is, longer than 12 months. Complete the required additional information for each subsection including:

• relevant settings

• disciplines needed to support the child

• evidence used to recommend that support is required for an extended duration.

Section 7: Additional information, if required

Only use this section if additional space is required.

Section 8: Early childhood partner statement

Consider all the information gathered from multiple sources, including your observation of the child and their interactions with other people. Provide your professional opinion based on the evidence detailed that the child will not likely or likely meet all the early intervention requirements - developmental delay (external).

For Internal Use Only Section 9: Important action before completing and submitting this form

Ensure the information in the EODD form has been discussed with the child representative(s).

Ensure you check the box declaring that the information contained within the EODD form is accurate and complete.

Insert name, electronic signature and date of the early childhood professional who has entered all information into the EODD form. If the early childhood professional who completed the EODD form does not meet one of the qualifications listed below, additional fields are provided for a supervisor’s name, qualification, signature and date, for co-signing. The supervisor must have a listed qualification.

Early childhood partners should have internal processes and professional development including supervision to support all staff who complete the form.

The EODD form is a fillable PDF. The fill and sign functionality within Adobe PDF secures the form once it has been signed. For this reason, once signed, the form can only be modified to include the supervisor’s signature. Ensure any edits required are completed prior to signing. Qualification list

Early childhood professionals can include allied health professionals and early childhood teachers and educators.

The list of professions below require an Australian Health Practitioner Regulation Agency (AHPRA) registration

• Occupational therapist

• Psychologist

• Physiotherapist.

The list of professions below must hold membership of the relevant professional association or be eligible for membership/registration:

• Social worker: Membership with the Australian Association of Social Workers

• Speech pathologist: Practicing membership with Speech Pathology Australia

• Developmental educator: Full membership of Developmental Educators Australia Inc.

(DEAI).

A person who has a bachelor’s degree or higher in Early Childhood Education, Primary Education or Special Education, who where applicable has a current registration or accreditation with their state or territory’s relevant professional body.

A person who has a diploma in early childhood that is recognised in the National Quality Framework list of approved qualifications (external).
