ADDENDUM: Case Study of Systemic Administrative Failure (May 2026)
My daughter (aged 6) has had her NDIS eligibility revoked, effective June 25, 2026, due to an alleged failure to provide a functional capacity assessment. I submit the following timeline as evidence that this “failure” is a direct result of Agency system blocks:

Agency Confirmation of Permanence: On March 19, 2026, an Agency representative (staff member ‘Lisa’) confirmed in writing/phone that my daughter’s diagnosis (Childhood Apraxia of Speech) is lifelong and approved her plan until 2028 based on this permanent status.

Proactive Compliance: I requested an extension for the required functional assessment in March 2026 to ensure clinical evidence was provided.

System-Induced Non-Compliance: The Agency’s mandatory automated “funding periods” blocked the payment of clinical invoices for the provider (Cerebral Palsy Alliance). Consequently, the provider could not be funded to complete the 15-hour functional report required by the Agency.

The Contradiction: The Agency has now revoked eligibility for non-submission of a report that their own automated payment system effectively banned us from paying for.
IMPACT ON INFORMAL SUPPORTS As a primary carer, I have attempted to resolve this through the Agency’s internal complaints and Ministerial referral processes. However, I am currently facing “carer collapse” due to the requirement to act as an unpaid administrative auditor, managing conflicting instructions from the Payments Branch, Planning, and the Portal support team. The Agency’s current legislative framework fails to account for the sustainability of carers when the Agency’s own IT architecture creates insurmountable administrative loops.
CONCLUSION AND RECOMMENDATION I request that this addendum be appended to my formal submission. This case study demonstrates that the proposed legislative mechanisms for “Securing the NDIS” are currently failing to protect participants. I urge the Committee to consider that without a mandatory IT audit and a fix for cross-period payment processing, the Bill will exacerbate, rather than solve, the current administrative crisis.
