Kumanjayi Johnson, a 45-year-old Arrernte man with severe cognitive impairment, walked away from NDIS-funded supported independent living (SIL) accommodation operated by Lifestyle Solutions in Alice Springs in December 2022. His body was found six days later; he had died from environmental exposure and dehydration.
The coroner identified compounding failures by the provider and NT Police that delayed the search. Lifestyle Solutions staff did not properly follow their missing-person procedure or provide police with the relevant missing-person checklist, and communication failed to convey the degree of risk and urgency. The findings also examine inadequate cultural and family involvement, and gaps in positive behaviour support and SIL practice.
