7.Select The Correct Option From Drop-Down List At Select Eligibility Criteria The Disablity/Condition Meets. Note:The disabilty may not meet eligibility criteria for NDIS funding.The eligiblility criterias are the disability or early intervention requirements.The disabilit will still be recorded even though its not related to participants elgibility for ndis.If this applies.select Does Not Meet Any Eligibility Criterias.

Step-by-step Instructions:

If you need update primary disabilties go section Select A New Primary Disability, if dont then step ten.

Step Ten: Save And Go To Section Add Evidence Documents.

Subsection 4.3 : Selecting new primary Disability

To identify your primary disabitly look at assessed impairment which usually has greatest impact on daily life.

Step One: Select arrow next Assessed Impairment that want make as a PrimaryDisability and select Edit. Step Two: Select checkbox of PrimaryDisability. Step Three: Enter today’s date in Start Date field, Step Four: If participant acquired date is known enter it OnsetDate based evidence such medical report otherwise leave blank Don’t End Date.

Note only add an enddate if no longer appropriate list disabley record example added error they don’t experience anymore.

5.Select The Evidence Type From Drop-Down List Then Click Save.*

Subsection 4.4 Adding Evidence Document

1.Select the Documents Tab, 2.Upload required evidence or link to case using articles: o What evidence disability needed? o Link evidence with article.

Record decision reason

You must record your reason for updating the participant’s disabilities.

  • If received a request via Enquiry case: o Added new primary disability: Tell requester they should close their Enquiry Case & contact participants with outcome; o Declined or requested more info on Enquiry: Inform them it may be kept open until additional evidence provided. Reassign this Enquiry Case back to Requester. Log an internal communication activity recording updates/reasons; use article Log an Activity Or Internal Note.

Review And Submit The Case

Select “Update Person Account” tab, then click Next. At “Review” stage check information carefully before submitting (click Previous if changes needed). Submit when correct and message will confirm successful submission/case closure.

Before you start

Impairment category information is called ‘Notice of impairments’ as per section 32BA of NDIS Act (the Act). A participant can apply anytime to change their impairment categories including adding/removing an impairment. The term “vary” used within legislation but we use “change” when communicating directly due its plain English nature; any alteration constitutes delegate decision-making under this act. Decisions regarding whether a variation should be made are reviewable according to law.

All staff members are able to submit requests through this feature. A designated reviewer will assess these submissions before deciding whether any updates should occur.
