Work Instruction: Concurrent Support Request

This is an interim work instruction and is subject to change.

This work instruction provides guidance for National Disability Insurance Agency (NDIA) staff (Health Liaison Officers (HLOs), Aged Care and Hospital Interface Branch Executive Staff).

Overview

It is expected that generally, health will provide all of the participants care needs while in hospital. There are some circumstances where a participant may access supports from their plan during admission. Where this occurs the NDIA requests that the health service provide advice on the requirement to access the plan and the adjustments the facility has made to provide an inclusive service. These are known as concurrent supports and recognised by the NDIA and State governments through the Applied Principles and Table of Supports (APTOS). If there is concern or evidence that concurrent supports are being utilised incorrectly, the HLO should discuss this with their Assistant Director (AD).

When to use the Concurrent Supports Register

The Register should only be used once the following has been completed:

  • Health Representative sends request to HLO, ensuring they detail:
    • The participants primary disability and complex care needs
    • The disability related reasons why the participant requires concurrent supports, including risk to self and others and considering use of informal supports as well as participant and family preferences (examples included below)
    • Reasonable Adjustments implemented by the hospital (examples included below)
  • HLO reviews the request, extracts detail relevant for consideration and makes a recommendation to the AD including hours, care ratio and duration and what the supports are intended to be used for
  • AD provides outcome – this includes hours, care ratio and duration and what the supports are intended to be used for – this may also indicate a review period

The concurrent support register should not be used prior to AD approval.

Completing the Concurrent Supports Register

Once an outcome has been decided, the approver (or Hospital Interface Branch AD) should enter the information into the register and send outcome email to HLO.

HLO to drag outcome email from AD into HD Case Activity tab (this will ensure this can be continue to be monitored in the event that the original HLO is not available) and add a note to HD case specifying that it has been added to the case activity, the outcome and review date (if applicable). Example HD case notes:

dd/mm/yy HLO logon – concurrent support <approved/declined/partially approved> until dd/mm/yy, concurrent support register completed by (include information if review is required). Concurrent support request and approval added to HD case activity tab.
