3 Understand the reason for a participant risk assessment
A participant risk assessment (PRA) can take place at any stage. It usually follows an interaction the participant has with the National Disability Insurance Agency (NDIA), or their my NDIS contact. This could be at a check-in or as part of the access process. You can create a PRA case once the NDIA has been notified of a change in the participant’s situation. You’ll record and update your answers in the PRA case, using the Guidance for Success hyperlinks to answer questions relating to:
- any current investigations or information about the misuse of the participant’s plan or previous plan
- the participant’s insolvency situation
- any signs of gambling
- any signs of substance abuse or historical record of substance abuse.
You’ll record the information you receive in a PRA case. A planner delegate will review and take the appropriate actions. The planner delegate can then help and support the participant. For example, the participant may need support with their plan and budget management.
You can create a PRA case:
- when preparing for a participant’s first plan
- at a plan meeting
- at a plan implementation meeting
- at a participant check-in
- at a plan change meeting
- when a participant leaves the NDIS.

4 Create a participant risk assessment case
- From the Person Account, select Cases.
- At the Cases screen, select New.
- At the New Case screen, select Participant Risk Assessment.
- Select Next. The Participant Risk Assessment case will display.
- From the Case Origin drop-down, select either Email, Phone, Web, Internal or In person.
- For all other options, use the default settings.
- Select Save.

5 Complete a participant risk assessment case

From the Person Account, select the relevant case hyperlink.

At the Participant Risk Assessment screen, review the information in the Details tab to make sure it’s correct.

Select the Participant Risk Assessment tab.

At the Participant Risk Assessment screen, select the relevant options at Has the participant who will manage the funding currently or in the past 5 years been insolvent under administration?
Note: The hyperlinks under Guidance for success will help you answer these questions and offer you further instructions. Select each relevant hyperlink to open a new browser.

Select Yes or No at Is there an investigation of a participant, child representative or nominee for misuse of or fraud in relation to plan funds, including not spending in accordance with the plan; or involving the NDIS more generally?

Select Yes or No at Does the participant have a gambling addiction?
Note: Don’t ask the participant this directly.

If Yes, at Please select the impact to the participant, select the appropriate options.
Note: you can select more than one option.

If No, go to step 7.

Select Yes or No at Does the participant have a history of habitual/continued substance abuse within the last 12 months?
Note: Don’t ask the participant this directly.

If Yes, at Please select the impact to the participant, select the appropriate options.

Select Next. The Please attach any relevant documents screen will display.

6 Attach or link documents to a participant risk assessment case
At the Please attach any relevant documents screen:

Select Add Documents or Link Documents at the Please attach any relevant documents screen.
- To Add Documents, go to step 2.
- To Link documents, go to step 10.

At the Document Details screen, select the relevant drop-down option at each of the following:
- Please select a Document Category
- Please select a Document Sub Category.

Select Yes or No at Does the document contain any Personal identifiable information (PII)?

Record any further information about the document in the Description free text field. For example, a brief overview of the document.

Select Next.

At the Upload Document screen, select Upload Files, or drag the document file, to upload the relevant documents.
Note: Make sure the file is named using the naming convention: YYYY-MM-DD – description of the document – author.

Go to the file location on your device or network directory and select Open.

Select Save.

Select Close.

At the Link Documents screen, select the relevant document using the tick box.

Select Link or Download from the Link Document screen.

Select Link to link it to the case record. Select Close.

At Download, select from the downloads options from your browser downloads list.
To check the documents have been linked to the case record:

From the Person Account, select the Documents tab.

At the Participant Risk Assessment Summary screen, review the information.
Note: If you need to change details, select Previous.

Select Submit.

The case will automatically move to Closed.
If you need to update or delete a document, go to article Update or delete a document from PACE.
