3 Check evidence for eligibility requirements

Verify evidence for List A, B, or D conditions

Who can provide the right evidence

A treating health professional must provide evidence of a List A, B, or D condition.

What should the applicant give us?

The applicant should give us evidence from a treating medical professional (such as a medical report relating to their disability) that is original, genuine and specific to them. If you suspect the evidence is not real or authentic, go to article Escalate application integrity issue.

The evidence must prove the applicant’s diagnosis is a condition from either List A, List B or List D.

How to check the evidence

Before making an eligibility decision, you must check the applicant has provided the correct evidence and it has been provided by an accredited and/or registered health professional. Go to article Check treating health professional details.

For all applications, you must also check if the applicant has an urgent situation. Go to article Request priority eligibility decision.

If you need more evidence to make an eligibility decision, then you will need to ask the applicant.
