my name is Frank Filardo.

My son has been diagnosed with :- 1. Intellectual Disability 2. Psychosocial Disability (Schizophrenia - Medication Clozapine) with continuous, persistent and complex needs as well as disabling symptoms and medication side effects 3. Alcohol and Other Drugs 4. Degenerative Immune System 5. Obesity 6. Borderline diabetes 7. Cholesterol

He was receiving support services from DHHS, Partners In Recovery (PIR), MIND and other organisations. Those supports and services were terminated on the 30/06/2016.

He was advised that he was qualified (DSR) and would be getting an NDIS package soon. The NDIS package was provided in September, following a phone interview.

He missed out on any supports and services from the 30/06/16 to the 27/09/16

Following the 1st phone Plan, which didn’t take into consideration all his requirements. A review was organised and a 2nd Plan was issued in February 2017

He missed out on any supports and services funded under the Plan from 27/09/16 to February/17 waiting for a Support Coordinator

He also missed out on any services and supports from Feb/17 to Sept/17 even though he had Support Coordination. He finally got some social support in Oct/17.

Most of the used funds (1st $48.000.00 nil) 2nd $65.000.00 (15%) have been mostly spent on Support Coordination and Funds Management.

     Plan included funding for housing, Specialist Disability Accommodation (SDA) and

support services coordination, Supported Independent Living (SIL).

    has all the assessments, as arranged by the coordinators.

He requested an NDIA review.

An appointment was made by an NDIA Planner. In attendance at the NDIA offices, as per NDIA Planner’s request, the Planner, and parents, and the service provider. The Planner’s had only arranged for the replacement of the Housing and Support Coordinators, following my continuous complaints about no/poor service ather than, what we thought was a service Plan review, dealing with and resolving the SDA and the SIL.

The Planner new Support Coordinator arrangement. The service provider office is at least one (1) hour drive away from where we live, so, what we’ll be looking forwards to is that there will be at least two (2) hours charged before any service is delivered.

The previous services provider was local, with an office in Diamond Creek.

So, in short, going into thirteen (13) months with an NDIS Plan, is left with a lot more unused funds in his Plan, due to the fact that he has NO CHOICE or CONTROL.

Rather than having access to the much needed supports and services and NO FUNDS.
