Evidence of Disability – Alternative Documentation specific to Psycho Social Disability
These instructions relate to the Proforma and Client Questionnaire documents developed by the ACT Health Directorate.
Here is how this documentation is to be used:

The first part of the ARF is completed as normal by the client

When you reach the EoD section of the ARF (p.5) you now have the choice to either a. Complete it as normal – ie. use what is provided in the ARF; OR b. Use the Proforma and Client Questionnaire instead Using the Proforma and Client Questionnaire works as follows:

Tick the first box in the EoD section of the ARF which says “copies of reports” a. To be very clear you might then want to cross-out the remaining parts of the official EoD section of the ARF form

The client fills out the Client Questionnaire – this is designed to assist the GP/Clinician to fill out the Proforma

With the assistance of the Client Questionnaire, the GP/Clinician fills out the Proforma a. The front page of the Proforma is to be completed by the GP/Clinician, and signed. b. The following 3 pages are to be completed by the GP/Clinician and/or the staff member associated with the relevant referring service, and signed.

Once completed, the Proforma only is attached (ie. do not attach the Client Questionnaire) to the ARF
